This is the misunderstanding that matters most commercially and clinically. A sweet labelled sugar free has had sucrose and glucose replaced — but the replacements are sugar alcohols, also called polyols, and polyols are carbohydrates. They contain calories and they are counted in the carbohydrate figure on the nutrition panel.
They are absorbed more slowly and less completely than sugar, so the effect on blood glucose is lower. Lower is not none. Diabetes UK are explicit that sugar-free confectionery is not a free food. Read the carbohydrate figure on the panel, not the claim on the front of the pack.
The names to look for are isomalt (sugar-free boiled sweets), maltitol (sugar-free chocolate and chews), sorbitol (mints and chews) and xylitol (mints and gum). Alongside them you may see sucralose, acesulfame K or steviol glycosides, used in tiny amounts and contributing essentially no carbohydrate.
Because polyols are incompletely absorbed in the small intestine, part of what you eat reaches the large intestine, where gut bacteria ferment it. That draws in water and produces gas. In plain terms: bloating, wind and loose stools if you go past your own tolerance, which varies a lot between people.
UK law requires the statement “excessive consumption may produce laxative effects” on foods containing more than 10% added polyols. It is a real warning rather than boilerplate. Maltitol is generally the most likely culprit; isomalt is generally the best tolerated. If you are trying a sugar-free line for the first time, start with a small quantity.
Tooth decay is driven by oral bacteria fermenting sugars and producing acid. Polyols are poorly fermented by those bacteria, and xylitol is not usable by them at all. That is why dental guidance treats sugar-free mints and gum differently from sugared ones, and it is the one place where “sugar free” is a health statement rather than a marketing one.
The corollary is less comfortable: a sugared mint sucked slowly over twenty minutes is close to the worst thing you can do to your teeth, because it keeps the mouth acidic continuously. Frequency and duration matter more than quantity. That is covered in are mints good for your teeth?
Most cautions about sweets and children are vague. This one is not. Small, hard, round sweets and lollipops are a genuine choking hazard for children under four — the shape and hardness are the problem, because they can seal an airway rather than being crushed. The NHS is clear about it, and it is worth knowing which formats are safer rather than avoiding sweets entirely.

Xylitol is highly toxic to dogs, at doses far below anything that would affect a person. If you keep sugar-free mints or gum in a coat pocket or a handbag at dog height, that is worth knowing. There is no equivalent risk to humans.
