Sugar-free does not mean carbohydrate-free, and the sweeteners used still count. Here is what is actually in them, and why the label carries a laxative warning.
We are a sweet shop, not a clinician, and nothing here is medical advice. If you have diabetes, the people to take dietary direction from are your GP, your diabetes team or a registered dietitian. What we can do is explain accurately what is in a sugar-free sweet, because that is our own product information and it is frequently misunderstood.
This is the single most important point. A sweet labelled sugar free has had sucrose and glucose replaced with something else — but the replacements are usually sugar alcohols, also called polyols, and polyols are carbohydrates. They contain calories and they are counted in the carbohydrate figure on the nutrition panel.
They are absorbed more slowly and less completely than sugar, which is why they have a lower effect on blood glucose, but “lower” is not “none”. Diabetes UK are explicit that sugar-free confectionery is not a free food and still needs to be counted. Read the carbohydrate figure on the panel rather than the claim on the front.

Alongside these you may find high-intensity sweeteners — sucralose, acesulfame K, steviol glycosides — used in tiny amounts to top up sweetness. Those contribute essentially no carbohydrate.
Because polyols are incompletely absorbed in the small intestine, a portion of what you eat passes into the large intestine where gut bacteria ferment it. That draws in water and produces gas. The result, in plain terms, is bloating, wind and loose stools if you eat more than your tolerance.
This is why UK law requires the statement “excessive consumption may produce laxative effects” on foods containing more than 10% added polyols. It is a real warning, not boilerplate, and tolerance varies a lot between people. Maltitol is generally the most likely to cause it; isomalt is generally better tolerated.
If you are trying a sugar-free line for the first time, start with a small quantity rather than a bag.
Here the sugar-free claim does something real. Tooth decay is driven by oral bacteria fermenting sugars and producing acid. Polyols are poorly fermented by those bacteria, and xylitol in particular is not usable by them at all. That is why sugar-free mints and gum are treated differently from sugared ones by dental guidance, and why they are the one category where “sugar free” is a meaningful health statement rather than a marketing one.

Xylitol is highly toxic to dogs, in quantities far smaller than are harmful to people. If you keep sugar-free mints or gum in a bag or a coat pocket at dog height, that is worth knowing. There is no equivalent risk to humans.

Sugar-free sweets are not a free food and are not a health food. For someone managing blood glucose they can be a lower-carbohydrate option than the sugared equivalent, but they still need counting, and eating a lot of them has a predictable and unpleasant digestive consequence. For teeth specifically, they are genuinely better. Anything more specific than that is a conversation for your own care team, with the nutrition panel in front of you.
